Oak Street Health opens Fort Wayne Center exclusively for adults on Medicare

FORT WAYNE—Oak Street Health continues to expand their service to Adults on Medicare in Indiana by hosting the Grand Opening of their first clinic in Fort Wayne. The Grand Opening event took place on Dec. 19 in their Fort Wayne center, 436 E. Washington Blvd.

Oak Street Health has reduced hospital admissions among its patients by 40 percent and is excited to expand its innovative approach to primary care in Fort Wayne.

Oak Street Health patients enjoy a wide variety of services: they are assigned to a Care Team consisting of a doctor, nurse, medical assistant, as well as a medical records specialist to take notes. Clinics are also staffed with insurance specialists who offer support with Medicare paperwork as well as social workers who can connect patients with other services.

Additionally, each Oak Street Health center is equipped with big community rooms offering daily classes, activities, and other programming.

Oak Street Health serves adults on Medicare regardless of income level and at no additional cost beyond their normal Medicare plan.

In response to dropping temperatures and dangerously cold conditions across the Midwest, Oak Street Health community centers will serve as warming centers for older adults to ensure that seniors are safe and warm, and to provide access to health resources and medical services. Very cold weather puts older adults especially at risk. Oak Street Health centers will be open as warming centers through the winter.

Effective immediately, each of the 19 Oak Street Health community centers—including the Fort Wayne facility—will serve as warming centers from 9 a.m. to 5 p.m., Monday through Friday.

Centers​ will be open to all seniors, and will offer them warm drinks and daily activities.

Founded in 2013, the company has grown to serve 22,000 patients across 19 centers throughout the Midwest. Oak Street Health has 700 employees.
